Forms, Waivers & Contracts
These documents are voluntarily given as templates. You will want to look them over, adapt as appropriate, and/or have a lawyer examine them for you (if applicable). These templates come from various sources and are for you to copy and use as you wish.
Business Plan

No matter if you are a single yoga teacher or run a full program keeping a vision of the future in mind is a good business practice. If you are the type who doesn’t like to write out words use the format to create a vision board or board on Pinterest or doodles to be hung on the wall by your desk or home practice spot.

These sample contracts are guides for setting up documentation with teachers a program works with and can be helpful guides for teachers who have never had to negotiate a contract before. The percentages have been left blank. Interviews with folks over the last couple years have shown a wide range of variance in how these are calculated. If one is teaching for a non-profit the percentage to the teacher may be less than in a regular studio.
Liability/Sign In

This particular form is set up to be used in a venue where multiple types of yoga are offered – mat, aerial, SUP and aqua are addressed. Be sure to include the correct parameters for the space and materials used. (e.g. is there a weight maximum for hammocks or chains used for holding hammocks for aerial? How about the boards you use for stand up paddle board (SUP) yoga. Will a certain body weight make the practice impractical?) Also be sure to have forms reviewed by a local business lawyer.

This forms can be used for one time events, outdoor gatherings where lots of paperwork could be troublesome. It could also be used with individuals who have filled out prior full forms and need to just reconfirm. Be sure to include all parties who may be liable.
General Forms

Even if a student says their injury is not a “big deal” you may want to have a record. Anyone who visibly injuries themselves while you are teaching or is part of a program you run is something to consider noting. Especially if there the student must leave the class. If there is someone who can have them give details right then it is best, otherwise be sure to have their phone number to catch up with them later.
